In addition to the attached registration forms, the following documentation required in order to complete the registration:

• Original Birth Certificate

• 2 proofs of residency (utility bill, mortgage statement, rental agreement)

• Immunization records (must be up to date)

• Universal Health Form – Physical must be within the last year (365 days)

• Custody paperwork (if applicable)

Once your registration packet is completed, please bring it into the Main Office with all required supplemental documentation.
